Request an NDIS Nutrition Assessment and Report Package
The NDIS Assessment and Report package includes:

2 x 1hr dietitian consultations
Each consultation is 1 hour of direct appointment time, with 27 minutes of non-direct care included for preparation, documentation and clinical review.

Appointments can be completed:
  • in person at our Sandgate clinic
  • via telehealth
  • using a combination of clinic and telehealth appointments

Your child must attend at least one of the two appointments. Families are welcome to have their child attend both sessions, however one session may also be completed as a parent/carer-only appointment. This can be helpful to allow time to discuss your child’s history, family priorities, challenges, goals and support needs in more detail.

1 x Detailed NDIS nutrition report
Following the assessment sessions, your dietitian will prepare a detailed NDIS report. Reports are usually 10+ pages and include clear recommendations to support NDIS decision-making and planning.

The report may include:
  • summary of your child’s diagnoses, background and current supports
  • nutrition and feeding history
  • growth, intake and nutritional adequacy review
  • current feeding, mealtime or tube feeding concerns
  • functional impact of nutrition and feeding challenges
  • risks related to inadequate nutrition, hydration, growth, feeding safety or participation
  • goals for nutrition therapy and capacity building
  • therapy recommendations, including suggested frequency and duration
  • links between nutrition support and your child’s NDIS goals
  • rationale for why dietitian support is reasonable and necessary
  • recommendations for home enteral nutrition, formula, supplements or consumables where required
  • quotes and estimated costs for nutrition-related consumables or resources
  • Assistive Technology recommendations where clinically indicated, including feeding pumps, pump stands, backpacks or other required equipment
  • recommendations for care team liaison, school/daycare support or multidisciplinary meetings where needed

Reports are provided to families as a draft first. Once reviewed and approved, the report is finalised and sent back to the family.

Fees
The NDIS Assessment and Report package includes:
  • Consultation 1: $260 - 1 hour direct appointment time + 27 minutes non-direct care
  • Consultation 2: $260 - 1 hour direct appointment time + 27 minutes non-direct care
  • Detailed NDIS Report: $760 - Includes 4 hours and 15 minutes of report preparation, clinical analysis, recommendations, quotes and documentation, family to provide feedback and then draft is finalised
Total package cost: $1,280

Timeframes
We can generally offer assessment appointments within approximately two weeks, depending on clinician availability and the urgency of your request.

Once both consultations are completed and all required supporting documents have been provided, the completed draft report is generally available within approximately one week.

This service may be helpful for families who need updated dietitian evidence within a 3–4 week timeframe for an NDIS plan review, funding request, change of circumstances, tribunal review, Assistive Technology request or other NDIA documentation request.

Documents to Upload Before Your Appointment
To help us prepare and ensure we can produce an accurate, detailed and useful report, please upload any relevant documents before your first appointment.

Helpful documents may include:
  • your child’s current NDIS plan
  • previous NDIS reports or therapy reports
  • reports from speech pathologists, occupational therapists, psychologists, physiotherapists or other allied health providers
  • summary letters from your paediatrician, GP or specialist
  • diagnosis letters or medical summaries
  • current medication list
  • growth charts or recent weights/heights
  • feeding plans, mealtime plans or tube feeding plans
  • hospital discharge summaries
  • formula, supplement or enteral feeding information
  • school, daycare or support team documentation
  • previous Assistive Technology approvals, quotes or equipment information

The more information we have, the better we can understand your child’s needs and prepare a report that clearly communicates the supports they require.

    Please note: Brisbane Paediatric Nutrition currently accepts Self-Managed and Plan-Managed NDIS participants.
    If unsure, please refer to your NDIS plan, discuss with your Support Coordinator or Plan Manager or please upload copy of current plan below and we can review to determine which code is applicable for your child.

    3. Reason for Requesting an NDIS Report

    This helps us make sure the report is targeted to your child’s funding or review needs.
    This helps us understand the purpose of the report and what evidence may be needed.

    4. Diagnosis and Medical Background
    E.g. GP, Paediatrician, Speech pathologist, Occupational therapist, Physiotherapist, Psychologist, Support coordinator, School or daycare, Hospital team or Connected Care Team

    5. Appointment Preferences
    This service includes two consultations. Your child must attend at least one consultation. Families may choose to have one session as a parent/carer-only consultation to allow time to discuss history, concerns and goals in detail.

    This may include family priorities, previous challenges accessing support, upcoming deadlines, school or daycare concerns, equipment issues, feeding safety concerns, changes in your child’s health, or anything else you feel would help us understand your child’s needs.
